What You Need To Know
Take the time to verify the accuracy of your personal information such as your address, dates of birth and eligible dependents and if you’re enrolling new dependents, don’t forget to be prepared with dependent verification paperwork to ensure they are enrolled without delay.

*If you’ve changed your password and have forgotten it, please use the “can’t access your account” option. Remember! You can access your benefits and enroll from any device.
Once your initial enrollment period has ended, all elections are considered final and cannot be changed unless there is a qualified life event.
What’s a Qualified Life Event?
Qualified life events (QLE) include, but are not limited to, the following:
- Marriage, divorce, legal separation, annulment or death
- Birth or adoption of a child
- Gain or loss of other health coverage
- Change in your dependent’s eligibility status
- Court order
- Retirement
You MUST submit documentation of a Qualifying Life Event (QLE) within 30 days of the event.
The benefits you elect will be effective from January 1, 2027 - December, 31, 2027, unless you experience a qualifying life event. Such events include birth, death, marriage, divorce, adoption, or a change in status, so choose your benefits carefully.
You become eligible for benefits on the first day of the month following your employment start date as a full-time team member scheduled to work 30 hours or more per week. You may also enroll your eligible dependents in some of the same plans you choose for yourself, including medical, dental, vision and certain voluntary benefits.
Eligible dependents may include the following:
- Legal Spouse (as defined by your plan)
- Domestic Partner (for details, refer to OZKInside – Human Resources – Benefits)
- Dependent Child(ren):
- Up to age 26 for Medical, Dental & Vision
- Up to age 19 (or 25 if un-married FT student) for Voluntary Life
- Any dependent who is unmarried, mentally or physically handicapped and meets certain documented disability requirements
NOTE: Benefits for domestic partners or beneficiaries who are not legal spouses or dependents as defined by the IRS
must be credited to the team member as imputed income.
Dependent Verification
Please be prepared to provide verification if you enroll a NEW dependent during enrollment or in the future. Any new dependent enrollment will require verification such as a marriage license, birth certificate or most recent 1040 tax document before your new dependent will be enrolled.

Under the OAP and HDHP
If dependents are enrolled, all charges applied to the individual deductible are also applied to the family deductible, but no participant will contribute more than the individual deductible maximum. When the family deductible is reached, no further deductible will have to be satisfied by any member for the remainder of the calendar year.
The information modeled here reflects the maximum your family could be out-of-pocket within a calendar year based on 100% in-network utilization in a worst-case scenario.
Please be sure that your doctors participate in the network in order to obtain the highest level of benefit.

Reliance Standard’s Accident Insurance provides a cash benefit based on the accident/injuries sustained. The benefit is paid directly to you, and you decide the best way to spend it. Why Do I Need Accident Insurance?

What Does it Cost?
Virtual health visits are treated the same as a visit with a primary care physician. Members on the OAP plan will have a $25 copay. You simply pay online at the time of service. Members on the HDHP will have a $60 copay - the HDHP plan covers 90% after deductible. Members on the HDHP plan will pay 10% after the deductible. Prior to the deductible, members will pay out of pocket for the cost of the visit. Virtual care primary care visits are generally the lowest cost form of care.
