We’ve partnered with MetLife and are pleased to offer a wide range of voluntary benefit options that offer you choice and flexibility, so you can take charge of your physical, financial, and emotional well-being.

Voluntary Benefits help to:

✔ Fill coverage gaps left by medical insurance and extend protection to lifestyle needs like legal help, identity theft protection, and pet insurance.

View Voluntary Benefit Options:

✔ Read the Voluntary Benefit Options page in the packet you received via mail, or

✔ Log in to Prism during your benefits enrollment period to view the specific voluntary benefit options available to you.

✔ In Prism, you’ll be able to review benefit details and rates before completing your enrollment online.

Aflac and Transamerica Life and Accident Plans

As part of our transition to MetLife voluntary benefit offerings, ProService will no longer offer Aflac plans or Transamerica Life and Accident Insurance plans starting with the 2026 plan year.

If you are currently enrolled in an Aflac plan and/or Transamerica Life or Accident Insurance plan, ProService will be mailing information directly to you with clear instructions on how to continue your coverage directly with the carrier, or transition to a similar MetLife plan.

Health Care Benefits

Please refer to Prism for the specific health care plans available to you. Click on the topics below to learn more.

Please log in to Prism during your benefits enrollment period to view the specific health care plans available to you.

In Prism, you’ll be able to compare plans side-by-side, review plan details, and view rates before making your selection.

If you want to waive health care coverage, you must check the “I agree to waive Medical” box in your HR Portal, and complete an HC-5 Waiver Form.

The HC-5 Waiver Form* will be sent to you electronically after the enrollment period has closed.

You must submit a new HC-5 Form every year to waive health care benefits and provide proof of valid coverage. (*applies to Hawaii employees only).

The health care coverage you select during your enrollment period will be effective January 1, 2027.  If your healthcare provider issues new cards, you should receive them by January.

If you’re happy with your current health care plan coverage and want to keep comparable health care plans for next year, no action is needed.

If you don't complete online enrollment, you will continue to be enrolled in comparable health care plans.

We highly encourage you to log in to your HR Portal and/or speak to a benefits consultant to review all of your benefit options, plans, and rates and make sure you have the benefits that will best meet your needs for next year.

Important: Some voluntary benefits need active enrollment every year, so it’s always a good idea to review all of your options.

Even if you don’t plan on making changes, we recommend that you review any applicable changes for the coming year and complete your online enrollment.

✔ If you do not complete online enrollment, you will continue to be enrolled in your comparable health care plan.

✔ If you want to waive health care coverage for the coming year, you must submit a new HC-5 Form (for Hawaii residents only), along with valid proof of coverage.
